This partnership includes Henry Ford Health hospital and clinical locations, as well as food pantries in the Gleaners network. Your care team consists of representatives from Henry Ford Health, United Way and food pantries in the Gleaners network. To coordinate your care, we will have access to the following information about you:
The information above will flow through multiple data systems at Henry Ford Health, United Way and food pantries in the Gleaners network. These data systems store client-level information. Only authorized users will have access to these systems and your information. We will only share information that is necessary to connect you with a food pantry and other resources. We do not share private medical information, such as your condition and treatment.
These systems have high security standards. All information you provide is confidential and will not affect any benefits you may already be receiving from government assistance programs.
