Our vision is a community where resources are easy to find and access no matter who you ask for help. We have the knowledge and the ability to make this happen by working together.
A Community Information Exchange (CIE) is a network that works together to better connect people with resources. Under community guidance, health and social services agencies make up this network. CIEs use technology to share information and help partners connect. This helps agencies grow their impact and individuals get the help they need faster and easier.
Closed-loop referral system is the basis of CIE. A closed loop referral has two parts: a referral is made and service is provided. Part two, whoever sent that referral sees the confirmation that a successful service was provided. Our 2-1-1 resource allows Connect4Care to easily organize and match services from client to agencies. This creates a more holistic, person-centered, data-driven care to the community.

Connect4Care™ results in better awareness of resources in the community through streamlined referrals and no duplication of efforts.
Connect4Care™ enables partners to better respond to trends and predict changes in community needs. This results in more informed and fair allocation of resources and policy solutions that strengthen the sector.
The CIE network is health and social services agencies in the metro-Detroit area. They provide services such as food, childcare, employment, transportation and housing. Each agency shares information about their services to further connect each client and agency in the network.
CIE is also community members who inform decision-making. Input is found in committees, feedback, and surveys and created into policy. There is no Community Information Exchange without community.
