Closed Loop Referrals

United Way for Southeastern Michigan, Henry Ford Health and Gleaners Community Food Bank of Southeastern Michigan are working together to meet your food needs. This partnership includes Henry Ford Health hospital and clinic locations, as well as food pantries in the Gleaners network. Read below to learn what will happen next if you decide to participate in this program.

  1. Your Henry Ford provider will notify a United Way team member about your food needs.
  2. A United Way team member will call you within a couple days to set up an appointment at a local food pantry.
  3. The food pantry will provide you with free food at their location.
  4. United Way will follow up to support you throughout the process and help with other needs you may have.

We expect this connection between community organizations to help meet your food needs. We also hope it will increase your access to other resources.

Data Sharing

To coordinate your care, we will have access to the following information about you:

  • Contact information, such as name, address and phone number.
  • Demographic information, such as race, sex and language.
  • Information about your needs, such as food, utility assistance and transportation.

The information above will only be used by authorized users at Henry Ford, United Way and Gleaners. The authorized users will not have access to any of your private medical records and will only share necessary information to connect you with a food pantry.

These systems have high security standards. All information you provide is confidential. This will not affect any benefits you may already be receiving from government assistance programs.
